Expand so you see the option for setting the available sites. you will need to first go into Window->Preferences->Install/Update. installing certain Eclipse updates is currently known to change the behaviour of CCS 5.1 (this is mostly things such as the default perspective, flash screen etc)
The best thing I can suggest is to upgrade CCS v5.1 () This version of CCS is based on Eclipse as it is and therefore accepts next to all plugins. Also note the plug in you reference is built for a newer version of Eclipse than that used so is quite possibly incompatible. This is because for this CCS version Eclipse has gone through slight modifications by us. First of all, as mentioned on the wiki page you found, CCS v4 has limitations when it comes to plug ins.
